Here is what you need to continue:

	Click the checkbox to signify you have, or will be getting copies of the following documents.
	Florida Driver License which reflects the property address. (If you do not have a driver license you must have a State of Florida Identification card for non-drivers only). Note: "Valid Only in Florida" driver license is not acceptable. Click here to change your address for your Driver License or Identification card.
	Florida Voter Registration. If you are not registered to vote, you must have a notarized and recorded Declaration of Domicile. Your Voter Registration or Declaration of Domicile must reflect the property address. Click here to register or change your address for your Voter's Registration. Click here to complete the Declaration of Domicile.
	Note 1: If you are not a United States Citizen, proof of permanent resident immigrant status (such as a "Green Card") is required.
